Pivot table using alteryx

I need some help to prepare an excel kind of pivot table in Alteryx. I have attached a sample input file and the expected output for reference. Can you please help?

Hi @KaustubhK 

The appear to be 2 versions of what you're looking for...possibly caused by a bug in the new Community.

The first of those is in the temp1.xlsx that you attached and looks like this

this is what was provided by @apathetichell 

The second version is this (outlined in black)

This is what you see if you preview the file in the community.  As you can see the results are quite different.  I'm not sure how the preview functionality works in the community, but it appears to be out of sync with the contents of the actual file. 

In any case, the attached workflow gives the second result.
