Assuming your Table field is non-unique, you can use an Append tool to create the Text-to-Table association you are needing. Immediately following the Append, you can insert your Layout tool. From there, depending on whether you are going to extend the workflow further, you can use a field to batch out reports. I cannot tell much form the workflow itself you posted what that would look like - maybe a Patient ID number of some sort.
