Calculate values from different columns

Hi All,

Good day! 

I have attached a file with the folllowing inputs :

Original Inputs : Input 1 and 2
Rolling Sheet : Created in Alteryx in order to have a link between the above 2 files

Transformed Output : The working I have achieved 
Output required : The format and working of the output as needed

Problem:

I need to calculate the values of the last 4 columns in the Transformed output sheet.

The final output can be as in Trasnformed output sheet or can be changed to reflect the format as the Output required sheet.
But the column headers as per the Output required sheet will beed to be changed every year based on data.

The claims are taken for different periods for each header : same year, next year, etc.

Request you to let me know how I can get the working for those 4 columns in a way that suits my requirement.
