Issue with Formula (endif)

Hi Team,

I am trying to add cloumn and provide me the data as per the codes.I tried formula tool and is working until if ioput endif to it

 code 10,11,20,21 woulf be seclending and rest is non seclending (including blank)

Attching the output I require and wiith the formula I have inputed, please suggest where I am wrong. Thanks

@DBhagia Change the data type of the field DTC REASON CD to numeric and tray again
