We are 4 users sitting together, and we often experience something that feels like a bug.
If we have a small flow with only 1 input - it sometimes dublicates a field, and then creates an error for having a dublicate field?

Lets when you created the workflow there was no field "Invoice Date" coming from the input data and you had renamed another column as "Invoice Date". But later your input data had the field "Invoice Date" which would create this error. Since 2 columns with the same name is there.
One action you can take is to deselect unknown fields from the select tool. This way even if "Invoice Date" reflects later in the data it wont be considered.
