Split Data in Single Row to multiple Rows

Hi,

I am trying to convert the below input file similar to the output file.

I was able to convert one column alone using Transpose, but couldn't do for both. Any suggestions on how to achieve this? 

Input:

Name	Dept	Sub1	Mark1	Sub2	Mark2	Sub3	Mark3	Sub4	Mark4
Sam	CS	DS	100	OS	90	C	87
Tom	IT	DS	100	C	96
