I am converting our MS Access processes into Alteryx and wanted to know how to create a form similar to what I have in MS Access. These forms in MS Access are sent to end users after the workflow completes and I am having issues trying to duplicate this in Alteryx.
See attached for my MS Access form that I would like to duplicate in Alteryx.

Unfortunately, there is no way of reproducing that in Alteryx. The only option is to use analytical apps that have a similar but limited user interface.

If you are trying to create a form where the user needs to input the data and it will automatically do things with it, as @fmvizcaino you will be using something like the Analytic App.
However, if you are just trying to create for example a PDF form where it will be sent out to people. You can definitely create something similar to it using the reporting tools.
