Convert Columns to rows of Grouped Data

I have table like this

Project	Team	Domain	RiskScore	Milestone	PSR

P4521	Team2	Domainport	5	2	7

What I am trying to acoomplish is to convert these columns into rows like this

Project	Team	Domain	Score Type	Score
P1234	Team1	DomainTech	RiskScore	3
P1234	Team1	Domaintech	Mlestone	2
P1234	Team1	DomainTech	PSR	4
P4521	Team2	DomainPort	RiskScire	5
P4521	Team2	DomainPort	Milestone	2
P4521	Team2	DomainPort	PSR	7

i have no idea how to achieve this.. I am very new to alteryx. Please help
