Use a cross tab tool, configured as below. Either convert your date field to a Date format prior to the cross tab tool using the datetime tool, or (if you want to keep it formatted the same), use a dynamic rename tool after to pivot to replace the underscores (that will replace the dashes. In the below, the dynamic rename expression is REPLACE([_CurrentField_],"_","-").
