Got really confused with the input data ;)

Here is my solution. For me 2nd part was little confusing, do reply if anyone felt the same.

After solution upload, was going through your solution and found that you used only 7 tools to get the result but I had used 8. I learnt that no need to use Summarize tool before CrossTab. Kudos!
