read scanned pdf file fields to output to table

I have scanned PDF files (see attachment) with fields I want to read and stack in a table.  We have the Intelligence Suite.  The attachment presents 'Parcel' data in top right corner and a table in middle of page.  I would like to get this data and stack in a table.  I've used image template and annotated sample with less than desirable results.  I know this should not be a difficult workflow.  Ultimately, I will create an inventory of templates that will be referenced for many individual scans creating one large table that will be output for import into a second system.  I spoke to folks at the Intelligence Suite desk las week but have not received a response.  Thanks to all in the community.
