Messy Data Cleaning

Hi, I have been trying to clean the data however, I could not get this. can anyone help how to do it?

It has two sheets "Dirty" and "Clean". Clean sheet is an expected output.

Certainly a number of data prep steps needed here. I approach this by first using the first three rows to derive field names for the other rows. Then combined and cleaned. Let me know if you have any questions.
