Help to Generate Report

Dear Team,

I want to generate report  based on the attached data.

In pivot I want Origin column with customer name based on CreatedOn date in ascending order.

In summary report only customer name and origin should get reflect

@vishwa1979 what is the input data? try the below if input data is similar to summary data.
