Guidance on Converting Rows to Multiple Columns

Hello Alteryx Community,

 

I have a question about transposing rows to columns, while maintaining one single row for all duplicate names.  The data file I’m working with includes duplicate names, but the description fee and posted amount listed are different (see the gold highlighted columns). What I want to do is have only one row for each unique record, but with multiple columns that reflect the four-description fee amounts and the corresponding posted amount.

 

I have attached a sample of the data format.  If someone can share some insight would be greatly appreciated.
