Data format to Single line

I am trying to fit the information in a single line (row)  based on Medicine Name Code and Product Code - Could you please assist me on how to fix this issue.

Please find attached excel sheet having two tabs,

1) Tab1 : Original  input 

2) Tab2 : Required output.
