Assistance with Pivoting Dataset in Alteryx

Hi,

As part of a project, every dataset to be consumed is formatted like this:

And I need it formatted like this:

This obviously needs more than just the pivot tool.  Is this something that can easily be done in Alteryx?  I've attached a sample dataset.

This solution also worked, so thank you for sending that through!  The first solution just appeared slightly more elegant but I guess that is very subjective.
