This was one of those tasks where the process is actually fairly simple. I used a total of three tools of my own. One tool to get all the data in one place. One tool to generate the output. One tool to remove the parts I did not need. 

Getting it to match exactly is fiendishly difficult because of things like typing and hidden characters and similar little fiddly bits.

Hence this would be another situation where I would really like to have the requirements specified in much more detail than they are given.
