This was a bit clunky, but I got what I think was being asked for.  (Not what was showing in the output sample, which didnt make sense to alot of people).

Hopefully this is ultimately what was being looked for.

Will be perusing some other completed samples for more elegant ways to do this, as mine is not slick at all.

Attached is my solution.

If anyone is having issues trying to determine what the end result should look like I believe I cross-tab'd the Output they gave us to give a better idea of the end result.
