Struggled a bit to get the map to look right. I left a couple of things a touch different, but got it.

Started out appending all the points into a single row, then realized that was a mistake. You need to basically union them all in the report map, so that you can add them as layers. Also, if anyone struggled as much as I did to find the labels, you have to add it here, before you can see it on the layers:
I would also note, you need to manually add the layers with the plus button before you can change them:
