Transforming data

Hello, 

I apologise if this is the wrong place to post but I need assistance. 

I have a set of data (this is a small sample)

currently at this stage, I need to transform it to look like;

everything I have tried hasn't worked. 

Please can someone provide some guidance? I have managed this in excel but the amount of formulas to pull data in nearly crashed it.
