Trying to Flatten a table

I am trying to flatten this table. I've seen other posts about it, but can't seem to put the pieces together. 

 

Below is the table that I'm trying to flatten and have it create a new column dynamically for the result date and result

This is what I want to create, with the dates going in ascending order.

Hi @SM66215 

 

Column names have to be unique, but the workflow would be something like this:

 

  • Transpose
  • Number the records within each ID
  • Crosstab back out
