Transform tools - having troubles

I've tried cross tab tool which i believe is tool I should use but i am having a hard time trying to achieve what i want. I have the below data set.

Here is the desired format. What can i do to achieve this - is there another tool other than cross tab tool? any help/tips appreciated - thanks in advance!

Hi @RNSEG  -- could you please expand on the data set you are working with and the end goal for the transformation you are seeking to achieve?

Just looking at the input you have, versus the output you want, I don't see the data points lining up. So I am a little confused what you are going for here.

Also, if you can provide an excel file, it would be much easier to troubleshoot with you -- Thanks!
