Problem with fuzzy string matching between 3 columns

Hi, I am fairly new to Alteryx and have been working on building a (for me) fairly complex model.

 

I face an issue with fuzzy matching where I wnat to determine the match score for two names at the same company - see table below.

See for structure of the input table attached (CEO is always the same person for a company, Supervisor sometimes is the same as CEO). The goal is to identify when Supervisor and CEO are the same person.

 

My output from fuzzy match only provides a score for a few companies but  its typically dozens of rows that are repeated (see screenshot attached). I have no idea why.

 

Would be very grateful for help on this.
