Output with header and sub header

Hello Everyone,

I have the following input:

Name	Date	In	Out
Employee 1	02/01/2016	8:40:54 AM	5:17:36 PM
Employee 1	03/01/2016	8:42:45 AM	4:30:38 PM
Employee 1	04/01/2016	8:47:05 AM	5:25:42 PM
Employee 2	02/01/2016	8:48:02 AM	5:17:00 PM
Employee 2	03/01/2016	8:49:39 AM	5:19:08 PM
Employee 2	04/01/2016	8:47:20 AM	5:31:10 PM
Employee 3	02/01/2016	9:00:35 AM	5:08:24 PM
Employee 3	03/01/2016	8:57:30 AM	5:03:49 PM
Employee 3	04/01/2016	9:00:19 AM	6:25:29 PM

However, I need to put it int he following format;

	02/01/2016		03/01/2016		04/01/2016
	In	Out	In	Out	In	Out
Employee 1	8:40:54 AM	5:17:36 PM	8:42:45 AM	4:30:38 PM	8:47:05 AM	5:25:42 PM
Employee 2	8:48:02 AM	5:17:00 PM	8:49:39 AM	5:19:08 PM	8:47:20 AM	5:31:10 PM
Employee 3	9:00:35 AM	5:08:24 PM	8:57:30 AM	5:03:49 PM	9:00:19 AM	6:25:29 PM

I have tried using the cross tab tool, but to no avail. Does anyone know of a way of getting this output?
