Need Help - Solving a Workflow Problem

Hello,

I have a file with some customers login information (see attached Input_Sample).

Desired outcome:

I am trying to build a test, where if a customer failed to successfully login for 5 times, they will not be able to successfully login within 2 minutes.

Problem description:

I have been able to build a workflow that is working for this test for the most part, however, for a few records, it's not working and I cannot determine the reason for those test to fail with this workflow. 

I have highlighted the account keys that the workflow is working in Green. And for the account keys the workflow is NOT working in Yellow. Need help to identify, why the workflow is failing in some instances.
