Multiple rows to multiple columns

I am working on a price list for customers for the past few years.  I have computed the ave price/customer/year (Column W in attached input file) besides the ave price per invoice/customer.  However, I am not sure how to move the ave price/year/customer to reflect it under the respective year column instead of a separate row for each year. 

Attached is the input file for my data as well as the output how I would like it to be.

 Any suggestion?

There is something else that I missed out while looking at the solution.  Where there is a more than one year customer ave price, how can I reflect it on the same row but under the respective year column instead of separate rows? If you could refer to the input file for customer ABC Ltd & XYZ Ltd.
