I have multiple tables in an Excel sheet and want to merge into a single table

Hello Alteryx community,

 

Newbie here and will appreciate some help.

I'm working with employee dataset that I need to prepare for analysis and visualization. Currently, I have an Excel sheet where employee data is shown as multiple tables within a sheet, each employee having their own table with their performance data (please see attachment). 

I would like to combine the employee data into one table with employee names as a field. Attached file shows sample input and desired output format. Please any idea how to do this Alteryx?
