Help building a Classification Model

I am trying to build a classification model to predict employee engagement, however, I have encountered difficulty.

Would appreciate some guidance.

Thank you very much for your willingness to help me. I am trying to determine whether the classification model can be used to predict employee engagement in a company. I have attached their sample employee engagement survey results, along with what I am trying to achieve.
