Excel - need items in a row to be Column Headers

Hello, 

I have attached a Test data set. I am trying to figure out how to get the values listed in the "Header" column to be listed as separate columns. 

Output that I'm trying to achieve - columns: 

1. Entity
2. M-Code
3. Value
4. Cumulative Beginning Balances
5. True-Up
6. Cumulative Ending Balances
