Creating forms

I need assistance with creating forms. Can Alteryx take a spreadsheet and create a form for each name and have it in this type of layout and export it a Word or pdf file?

Attached is an example of the spreadsheet.

The Render tools can export your data as Word or PDF format, with the rest reporting tools, you should be able to make "similar" format.

@Qiu thank you. Do you know how I can create the layout above? I haven't done any reporting so I'm a little lost. Would I use the Horizontal layout?

You’ll need to do a few Tables and Renders - then merge them together to design it the way you want. 

Alternatively, if you only have 1 report, you can use the excel as a template and Alteryx can output to specific ranges - therefore keeping your original design and having Alteryx just pasting the values for you as is.

To do your whole template with its exact placement and quirks, that will take some time and effort.
