Create a PDF

Hi guys, 

So I want to create a one blank PDF file with mailing address in the middle of the page.

with this format 

I have all the address in an excel file, and I want the PDF name to be their employee number.

The excel file looks like this

 How can do that?

Have you tried using the Reporting Tools?

If you use a couple of Report Text tools this might be something easy to accomplish
