There are a few ways to do this. Here's one example using the tile tool and crosstab. This would be dynamic if there are ever more than 3.

1. Tile tool determines how many medicines are there for a given column A value & date
2. Formula tool builds the header
3. Crosstab formats how you want it

I've done this using the Tile tool

By the way, it's better to provide tables or attachments with your sample data rather than images.  That way people can download or copy and paste the data rather than having to type it out.
