Assistance Required - Finding the Unique Values + Adding incremental Numbers

Greetings,

I have 2 tables to create for which I am looking for some assistance with 1 Table (Output 1). I would really appreciate any guidance. I am not sure at this point even which tools to use. I tried using multiple Unique tools but all in vain. Please see the attached Excel sheet for more clarity.

Problem:

With the data firstly, I want "0000" in an Address Name column for a supplier but if the address changes for that supplier the number should change to "0001" and so on if there are more addresses for the same supplier. For the next supplier, the number should again start from "0000". Please see the desired result in the output sheet of the attached file. (Output 1)

Once it's done, I'll concatenate Address Name & Entity and will use the Unique tool to find all the unique values for supplier sites and I'll have the output 2.
