Appending User Input to a column

Hi, I am trying to append the user prompt input and another column values to a new column along with today's date. I have attached the image of the input file and image of how output should look like. I have also attached a sample workflow (as YXMD file, along with it's output screenshot)

The one thing I am not able to implement is appending the user inputted initial in between the other 2 fields.

Can somebody help me please?
