Append the column name to the current value if the value is none blank/non null

I have a table with 100 fields. I need to update Fields 4-100. I want to add the column name to the cell values that are none blank/non null. Do I have to use a macro in order to do this.

 

I was able to do the task with python pretty easily however my firm has restrictions writing python files
