header not outputting - fixed file

Hi - I have a tab delimited file that needs to be output as fixed, and there are hundreds of fields.  So i followed the advice to pull file in as delimited and a select with each field and the desired length.  this worked, however on the output file when i select is as flat ascii, it is not outputting the header, which is needed.  I am not sure what i am doing wrong, but am having trouble figuring this out when it seems like it should be pretty simple. Anyone?

Can you share an example workflow, or at the very least a screenshot of your output configurations?
