How to create Dynamic Columns

Hello!  I have a dataset that has a column for notes that get entered every time a case is updated. Sometimes the notes are too big to be concatenated into a single cell. To eliminate duplicate rows for the same Case ID I am looking for an option that will create a new notes column and place the next note in that cell, but I can't seem to figure this out!  Any help would be appreciated!  As I have other fields other than the NOTES column as in the example, if the solution would work with other similar type columns as well, that would be ideal!
