Creating a Pivot table

What is the best way to create a pivot table that has "types" of data grouped and then the count of specifics for that group? I need to create a table that has 2 insurance plans that groups the count of each specific status for that payer.

I have tried the summarize tool but I cannot figure out how to get it to group the payer type. I cannot get the sample information to attach either.
