Add a new column from a formatted report

Hi Guys,

I have an existing report that I am having to reformat into a usable table. The current report is structured so the plan number appears as a row with the relevant records for that plan listed below the plan name. This is repeated for each type of plan. So the general data looks like the 1st 4 columns in the image below. The highlighted row contains the plan name and each record below that plan name are those utilizing that plan. Below is an idea of what i'm working with.

What i'm trying to accomplish is to remove the row that contains the plan name and add a column that will have the plan numbers align correctly by row. See the image below for what i'm trying to accomplish. Any help or ideas would be greatly appreciated.
