Formatting Table for Export - Groupings as Headers

I have a standardized report that I'm trying to replicate in Alteryx.  I have data grouped at a few different levels, but I'm struggling to turn the grouped data into a formatted report.  Here is the layout of the data as it is pulled in:

From here, what I need to do is create a report that shows each individual County, City/Zip Code, and Provider Specialty, and then each Name/Address combination that falls under that grouping.  Here is an example:

There are some other formatting things, but those are easy.  Where I'm struggling is how to turn my groupings (County, City, Zip, and Specialty) into individual headers.  Thanks
