Fun challenge to solve. Used the text to column to parse age. Used the append tool followed by cross tab to get to get rating for each question and selected only the required columns then used the find and replace to look up with respondant ID.

I could see many people using FILTER tool (thus, manual change of parameter). I did the same and all other tools I had to use were APPEND , REGEX , DYNAMIC RENAME and SELECT
