Here is my submission, the first try in a vertical workflow.

My solution.  No Append fields tools used!

 I used the fact that binary numbers give you a "is this item there" representation. Generate all the numbers from 0 to 2^(number of items to choose from)-1.  Convert to binary.  Find all the records that have the number of 1's equal to the combo size.  Join back to the input names
