My solution is similar to most of the others, except I added a Text Input tool with the adder and multiplier for each factor score.

Here is my solution. I needed some time to figure out that one of the questions was cut of due to the field length of the string and that's why it didn't matched and I got a slightly different value for "conscientiousness".
