I didn't like using non-distinct values as keys... Made the assumption that the months will always be in order.

So for this one I've gone with creating an index of the months to get around the issue of the months not being definite (multiple M, J and A) followed by a Mod function to reset every 12 lines.  I've used the Max function in a multi row to populate the missing years and then a find replace to look up the index and replace with the month name.

Completed using record, formula and multi-row formula tools only. Assumption here that the input data stays correctly ordered. This would not work if the months were in random order.
