My solution:

I must say, that file was quite evil! Not sure if an error wouldn't have slipped through without the outcome the compare with.
The logic wasn't that hard, but to find and catch all exceptions took me about an hour.

Very good exercise for handle real messy data. There are a lot of going backwards to modified the formula to catch more exception cases, in particular the phone numbers. 

 

For the final answer, one missing match is about fax, which I cannot be bothered after so many steps 😂. Three cases are likely to be phone numbers, but the answer didn't parse out.
