Crosstab or Transpose? Or both?

Hey Guys, 

I am trying to get the output shown in the ALteryx screenshot to look like the excel screenshot. I have tried a myriad of things but have not been able to come to the right final product. I would really appreciate your guys' help.

Use Crosstab.  Group by Month.  Change Column Headers = Source Group.  Values for new columns = Count of MRN.  Method for aggregating values = Sum (or First).
