I was able to prep up a workflow for the requirement.

I am able to get the same format but seems like the average in output is not actually a average.

It is working fine for me try to drag and drop the input file before the multi field formula.

Try replacing your input source with the one in the workflow.

My Alteryx is 2020.1 if you want a lower version let me know i can downgrade it for you.
