II_Init Error while pushing data into SQL Table

I have a workflow that transforms data and outputs to SQL table using Data Stream IN tool and put into SQL Table using Write In Data DB Tool. I receive the following intermittent error when outputting to SQL table.

I checked the number of columns are 33 in both Input and Output table structure with same column headers.

In Data Stream In tool, creating Temporary table and appending the data into SQL table with latest date snapshot. I ran this workflow successfully few days back but it is failing now.

Can you provide some screen shots of the error and the workflow, as well as the tool config for the tool you are using to write to the table?
