How to transpose a header into row but it's more challenging than I thought - Need help!

In the attached file, I have created a hypothetical data set of how I want to utilize alteryx to clean up a dataset.  If anyone have any ideas on how to most efficiently tackle this - please let me know.

 

In the file - you'll see 2 tabs

[1] dirty data

[2] How I want it to be

I used a few Sample and Filter tools to get to just the necessary fields, but the real transformation was performed by a Dynamic Rename tool and a Multi-Row Formula tool.
