In order for the Community to help you with this issue, They will need to see your Action Tool setup. That is the tool that will be making the changes to the Input tool and would typically be where the issue is occurring. If the workflow runs on it's own without the app, but doesn't when you update it with an app, that usually means that your Action tools are not configured correctly to update the Input Tool.
